Cause

If you require all users to authenticate, but do not select an authentication method, all access will be denied.

Solution

If you want to force user authentication, select an authentication method. Otherwise, clear the Require all users to authenticate checkbox on the Web Proxy tab of the network properties.

If you want to use Basic authentication, consider enhancing security by using an SSL connection to send the credentials.
