Respond to a questionnaire [AX 2012]

To view a list of questionnaires that are available for you to complete, use the Questionnaires to complete page. Questionnaires can be used to gather information for a variety of purposes, such as testing skills of employees and applicants, evaluating a training course, or surveying employee or customer satisfaction.

Questionnaires must be created in the Microsoft Dynamics AX client application.


Depending on your role or the privileges that are assigned to you, you might need to go to your Employee services site before you complete the procedures in this topic.

  1. Click Questionnaires on the top link bar, and then click Questionnaires to complete on the Quick Launch.

  2. Select the questionnaire to complete.

  3. On the Action Pane, click Start.

    Depending on how the questionnaire was set up, the following buttons might be available:

    • Overview – Display all questions in a list.

    • Show note – Display or hide a box where you can enter text, such as additional information to say more about an answer.

    • Back – Return to the previous question.

    • Forward – Go to the next question.

    • End – End the answer session and save your answers.

    • Cancel – End the answer session without saving your answers.

  4. Type or select your answer for the question that is displayed and then click Forward to view the next question.

  5. When the last question is displayed, type or select your answer for the question and then click End to finish the questionnaire.

If a questionnaire must be completed within a time limit, you can end it in one of the following ways:

  • Complete the questionnaire within the time limit, and then click End to end the session.

  • Answer all mandatory questions within the time limit. When the time limit expires, your answers are automatically saved.


If you do not answer all mandatory questions within the time limit, your answers will not be saved when the time limit expires.
