Personal contacts (form)

Click Human resources > Common > Workers > Workers. Select a worker and click Personal information, and then click Personal contacts.

Use this form to create personal contacts and to designate them as emergency contacts, beneficiaries, or dependents.

Note

Only users in the roles that are selected in the Private location security area of the Global address book parameters form have access to view information about workers’ personal contacts. For more information, see Global address book parameters (form).

Note

This control is not available if Microsoft Dynamics AX 2012 R2 or AX 2012 R3 is installed.

Identification

Add identification information for the contact. For example, driver’s license, passport, or tax ID.

Modify the selected address or contact information.

View the selected address on a map.

Select the action to perform for an address record.

In Microsoft Dynamics AX 2012 R2, select from the following options:

  • Remove – Mark the selected address record as inactive.

    You can delete all inactivated addresses by running a batch job in the Delete inactivated addresses form.

  • Set defaults – Select the default address.

  • Advanced – Open the Manage addresses form.

In Microsoft Dynamics AX 2012, select from the following options:

  • Remove – Delete the selected address record from the list.

  • Deactivate – Set the status of the selected address record to inactive. The address record is not deleted.

  • Advanced – Open the Manage addresses form.

Select the action to perform for a contact information record.

In Microsoft Dynamics AX 2012 R2, select from the following options:

  • Remove – Delete the selected contact information record.

  • Set defaults – Select the default contact information record.

  • Advanced – Open the Edit contact information form.

In Microsoft Dynamics AX 2012, select from the following options:

  • Remove – Delete the selected address record from the list.

  • Deactivate – Set the status of the selected address record to inactive. The address record is not deleted.

  • Advanced – Open the Edit contact information form.

Fields

Field

Description

Name

The name of the personal contact or organization.

Relationship

Select the relationship of the personal contact to the worker.

Emergency contact

Select this check box to assign the contact as an emergency contact.

Primary/secondary

Select the emergency contact as a primary or secondary contact.

Note

This control is only available if the Emergency contact check box is selected.

Beneficiary

Select this check box to allow this person or entity to be enrolled in a benefit as a beneficiary.

Dependent

Select this check box to allow this person to be enrolled in a benefit as a dependent.

Gender

Select the gender of the dependent.

Note

This control is only available if the Dependent check box is selected.

Birth date

Enter the birth date of the dependent.

Note

This control is only available if the Dependent check box is selected.

Full-time student

Select this check box if the dependent is a full-time student.

Note

This control is only available if the Dependent check box is selected.

Person with disabilities

Select this check box if the dependent is a person with disabilities.

Note

This control is only available if the Dependent check box is selected.

Verification date

Enter the verification date supplied by the IRS for the disability.

Note

This control is only available if the Person with disabilities check box is selected.

Court ordered support (QMSCO/QDRO)

Select this check box if a court order requires that the worker cover the health care of the dependent.

Note

This control is only available if the Dependent check box is selected.

Expiration date

Enter the date that the court order ends.

Note

This control is only available if the Court ordered support (QMSCO/QDRO) check box is selected.

Domestic partnership

Select this check box if the dependent has a registered domestic partnership.

Name or description

The description of the address or contact information type.

Address

The location of the contact, as written or printed on mail for delivery.

Purpose

The purpose that was assigned to the address when it was created, such as “Home” or “Business.” One address can have more than one purpose. For example, the third-party shipping and invoice address can be the same.

Primary

This check box is selected if the selected address is the primary address that correspondence should be sent to for this contact.

Private

Select this check box to prevent the address from being visible in other modules.

Type

The type of contact information displayed. For example, Phone or URL.

Contact number/address

The telephone number or address of the contact.

Extension

The contact extension, if applicable.
